The Wellstar Foundation recently funded the FindHelp Integrated Social Determinants of Health Assessment Tool. Currently used by over 600 Wellstar team members, the tool helps connect patients to food, housing, transit and healthcare resources. While Wellstar uses varying data collection methods for patient social determinants of health (SDOH) assessments, integrating the FindHelp platform systemwide will support consistent social care referrals, data collection and entry into Epic and allow us to evaluate the tool to improve patient outcomes continuously.

Additionally, LiveSafe Resources recently received a 3-year funding renewal for the Sexual Assault Nurse Examiner program (SANE). Since 2015, Wellstar and LiveSafe, a benchmark sexual assault program in Georgia that serves as a model for other communities developing care resources and programs for victims of sexual assault, have provided nursing services for victims of sexual abuse in Cobb, Cherokee, Paulding and neighboring counties. This renewal of funding will supplement a portion of the program’s staffing costs and materials, reinforcing Wellstar’s commitment to providing personalized care close to home.
